How to file an incident postmortem

1. Create repos/<repo>/specs/<incident-slug>/ with spec.md, plan.md, tasks.md, evidence.md per the Spec Kit convention.
2. The evidence.md captures the timeline, root cause, fix, prevention.
3. Link from this page (hand-authored).
